1. Who we are
This website is run by Glenview Pharmacy and Glenview Medical & Walk-in Clinic, 2132 King Street East, Hamilton, ON L8K 1W6. Each is a health information custodian under Ontario's Personal Health Information Protection Act, 2004 (PHIPA) and protects your information as that law requires.
2. What we collect through this website
We only collect what you choose to enter in our contact forms:
- Your name, date of birth, phone number and email address
- How you'd like us to contact you
- Details of your request, such as prescription numbers, medications, a minor ailment you'd like help with, your current pharmacy (for transfers), a delivery address, or a preferred appointment time
- Anything you write in the message box
We do not ask for your health card number, payment details or passwords online. Please don't include them in a message.
3. How we use it
Only to respond to your request and provide your care, for example to fill, renew or transfer a prescription, assess a minor ailment, arrange delivery, book a visit, or register you as a new patient. We use the contact details you give us to reach you about that request.
4. How it is sent and stored
- Everything you send is encrypted on its way to us (HTTPS).
- The website does not keep a copy. Your request is turned into a document and delivered straight to the pharmacy's or clinic's secure inbox.
- Our team then files it in your pharmacy or medical record, which we keep for as long as Ontario law and our professional colleges require.
- Only staff who need it to help you can see it.
5. Who we share it with
We never sell or rent your information. We share it only when needed for your care (for example with your previous pharmacy for a transfer, or with your doctor), when you ask us to, or when the law requires it.
We use a few trusted service providers to run this website:
- Cloudflare: website hosting, security and delivering form messages to our inbox
- Google: our email inboxes, the clinic's New Patient registration form (Google Forms), maps and website fonts
- Medimap: the live walk-in wait time shown on the clinic pages
- Pharmabook: online vaccination booking (when you choose to book there, their privacy policy applies)
6. Cookies and website statistics
We don't use advertising or tracking cookies. To understand how many people visit and which pages are useful, we use Cloudflare Web Analytics, which doesn't use cookies and doesn't identify you. Embedded services such as Google Maps and the Medimap wait-time widget may set their own cookies under their own policies.
7. Please don't use the website for emergencies
Website messages are read during business hours. For anything urgent, call us at (905) 543-9797. In an emergency, call 911.
8. Your rights
- Ask to see a copy of your personal health information
- Ask us to correct anything that's wrong
- Withdraw your consent for us to contact you
- Ask questions about how we handle your information
